Linwood Village is a leading long-term care and skilled nursing facility dedicated to providing exceptional care and support to our residents. We are looking for dedicated and compassionate individuals to join our team and make a meaningful difference in the lives of our residents.
We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide high-quality care to our residents. The LPN/RN will be responsible for delivering direct patient care, monitoring patient health, and collaborating with other healthcare professionals to ensure optimal patient outcomes. The ideal candidate will have a strong commitment to patient care, excellent communication skills, and the ability to work effectively in a team-oriented environment. This position is Monday through Friday with 8 hour shifts.
Key Responsibilities:
- Provide direct patient care, including administering medications, wound care, and treatments as prescribed by physicians.
- Monitor and document patients' vital signs, physical and emotional status, and response to treatments.
- Assist with the development and implementation of individualized care plans in collaboration with registered nurses and other healthcare providers.
- Perform routine assessments and report changes in patients' conditions to the healthcare team.
- Educate patients and their families about health conditions, treatment plans, and self-care techniques.
- Ensure proper infection control practices and maintain a clean and safe environment for patients.
- Assist with activities of daily living, including bathing, dressing, and mobility support.
- Participate in interdisciplinary team meetings and contribute to care planning and problem-solving.
- Adhere to facility policies and procedures, as well as state and federal regulations, including HIPAA compliance.
Qualifications:
- Current and valid Licensed Practical Nurse (LPN) or Registered Nurse (RN) license in Oklahoma.
- High school diploma or equivalent; completion of an accredited LPN or RN program.
- Previous experience in long-term care or skilled nursing is preferred but not required.
- Strong clinical assessment and critical thinking skills.
- Excellent interpersonal and communication skills, with a patient-centered approach.
- Ability to work collaboratively in a team environment and adapt to changing patient needs.
- Physical stamina and strength to handle the demands of the role, including lifting and assisting patients.
