UPMC Your Care is hiring a Triage Nurse. You will be working Monday-Friday daylight hours Rotating call coverage throughout the week and rotating every 3-4 weekends. Call Coverage includes weekday evenings and weekends

Provide triage services for the UPMC Your Care program by conducting member and family assessments by protocol, researching clinical situations to educate the caller on the seriousness of the condition, and directing member care and care coordination in a defined period to be provided by the UPMC Your Care interdisciplinary team. Triage UPMC Your Care calls from members of primary care providers, specialty care teams, UPMC Health Plan, and community resources (UPMC Your Care Program Stakeholders), including providing programmatic education, as necessary. Support the information needs of the UPMC Your Care interdisciplinary team who will be responsible for determining additional follow-up care after triage.

Responsibilities:

* Ensure member safety by working collaboratively in addressing calls directed from UPMC Your Care Program Stakeholders, to triage members appropriately to ensure safe, quality patient care in the most appropriate setting, and to ensure notification to the UPMC Your Care interdisciplinary team by supplying timely notification and documentation on the member's disposition. Respond to calls from members/family members and UPMC Your Care Program Stakeholders, in a professional and timely manner. Assess the caller's needs; determine the seriousness and extent of the member's conditions, concerns, and current or progressive member status. Direct care and triage by established protocols to support the UPMC Your Care interdisciplinary team, which allows for ultimate clinical decision-making by the nurse. Assess the actual caller to determine the caller's understanding of the seriousness and extent of the member's current condition and the caller's ability to carry out all instructions provided by the UPMC Your Care interdisciplinary team.

* Notify the member's UPMC Your Care Primary Stakeholders as appropriate, review telephone assessment, and obtain follow-up instructions as needed. Generate and send follow-up reports and other correspondence to the UPMC Your Care interdisciplinary team, and to any identified UPMC Your Care Primary Stakeholder as appropriate. Perform daily surveillance of UPMC Your Care members to determine member's location/service utilization on the continuum of care. (i.e. ED, hospital, SNF) Serve as a community resource for other clinical consultations and appropriate triage and advice. Consult the program medical director for consultation regarding arising clinical concerns. Assist with methods to track department use rates, determine user satisfaction of the telephone triage/advice line, and to design and implement program enhancements. Work cooperatively with the UPMC Your Care interdisciplinary team, triage nurses, and UPMC Your Care Program Stakeholders to operate and enhance an effective market-responsive telephone triage/advice line.

Qualifications:

* Graduate from an accredited School of Nursing.

* 1 year of experience in triage nursing, preferably in an emergency department or physician practice or other medical call center preferred.

* Previous triage experience preferred.

* Ability to provide clinical advice.

* Highly developed analytical skills necessary to assess the clinical care needs of members, to assist with research and disseminate advice to minimize health risks to members, and to assist with evaluation of the adequacy and success of member telephone triage/advice practices.

* Advanced interpersonal skills necessary to query callers in a constructive way, to interact sensitively with sick and concerned patients and caregivers, and to collaborate with interdisciplinary members of UPMC Your Care, medical community, and program stakeholders.

* Ability to prioritize and perform effectively under the stress of urgent situations.

* Experience with providing care to frail, clinical complex individuals with social determinants of health, including care of the older adult preferred.

* Strong organizational and problem-solving skills with ability to make decisions independently.

* Self-motivation, enthusiasm, and creativity required.

Licensure, Certifications, and Clearances:

At time of hire, must have one RN licensure in PA.

* Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)

* Registered Nurse (RN)

* Act 33

* Act 34

* OAPSA

* Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
