Job Description:

Essential Functions/Responsibilities:Monitors and responds to triage calls in person and/or on the nurse phone line.Works closely with call center registration team and clinic team to deliver exceptional patient experience.Talks directly to patients on the telephone and then directs them to emergency rooms (ERs), walk in clinic, and home care advice or to schedule patients to their physician during office hours.Determines urgency of seeing the patient based on brief assessment and on familiarity with a patient's condition and history.Sends patients with high-risk chief complaints such as chest pain, abdominal pain, or severe headaches to ER immediately.Arranges appointments for patients who do not need to go to ER but need to see a physician. Consults with physician as needed.Telephone triage by providing and documenting advice for home care of illness/conditions; conducting follow-up phone calls regarding patient's health and progress as necessary.Provides patient education regarding disease processes, therapies and healthful behaviors.Assist provider and other clinical staff with obtaining basic vital information preparing patient for examination, treatment and minor surgical procedures.May be required to draw and collect blood and other samples from patients and prepare specimens for laboratory analysis.Administer vaccines and ordered medications by oral, IM, SQ or ID routes.Responsible for tracking system for follow-up appointments related to office visits, laboratory tests and procedures to regularly follow patients monitoring disease status.Schedule referrals for patients with specialist or outpatient as requested by the provider. Complete appropriate forms or requisitions needed for referrals or lab testing, including appropriate CPT/ICD-10 coding.Provide appropriate and timely documentation in the patient's electronic health record using standardized workflows and processes.Demonstrates competence in the clinical skills necessary to carry out assigned job duties.Thoroughly understand testing (lab and procedures) values and relationship to patient status with accountability to take next steps, as appropriate to scope and licensure.Provide education to patients and families using the teach-back method per protocols and guidelines.Serve as a clinical resource and professional role model for other nursing staff.Prioritizes work tasks and services, and completes tasks in a timely manner.Demonstrate initiative to improve quality and customer service by striving to exceed customer expectations.Perform other duties as requested by Rural Health Clinic Manager to facilitate the smooth and effective operations of the office.Demonstrates competence in the clinical skills necessary to carry out assigned job duties.Maintain compliance with CLIA, OSHA, Safety and Risk Management guidelines.Maintain compliance and behave in a manner consistent with all policies and procedures, including but not limited to Compliance, HIPAA and personnel workplace rules.Behave in a manner consistent with all Compliance and HIPAA policies and procedures.Be aware of what is happening in registration, clinic and the organization by attending department meetings, reading emails and regularly checking information on the organization's intranet site.Balance team and individual responsibilities; be open and objective to other's views; give and welcome feedback; contribute to positive team goals; and put the success of the team above own interests.QUALIFICATIONS:Graduate of an accredited program for Registered Nurses (ADN).Current license to practice nursing in the State of Iowa for Registered Nurse.BLS and Mandatory Reporter certification.Previous clinical experience preferred.Previous EPIC experience preferred.Customer/patient focusedStrong interpersonal skills.Ability to work as a team member.Strong verbal and written communication skills.Ability to write, read, comprehend and speak fluent English.Ability to understand and apply guidelines, policies and procedures.Strong computer skills.Critical thinking skills using independent judgment in making decisions.This position is Monday-Friday 8:30-5pm and will work in the call center.
