Demonstrated knowledge and competency in mental health serviceprovision.Experience/Knowledge: Must have course work and or experience inthe areas of cultural diversity, human development, etiology and treatment ofmental illness, alcohol and drug abuse, physical and sexual abuse, suicide, andintellectual disabilities. One year experience preferred. Experience workingwith children is preferred although applicants may have other skills andexperiences that could accommodate this position.Physical/Emotional/Social-Skills/Abilities: Ability to effectively and ethically counselchildren and families. Ability topresent professionally and work within a team format to plan, implement, andevaluate successful interventions. Ability to effectively run and process groups with children andfamilies. Ability to work within a teamformat to meet positive goals for children and interface with other agenciesinvolved in the ecology of the family. Must have mental ability to exercise sound judgment under pressure. Ability to exercise effective decision-making,ability to set and demonstrate appropriate boundaries, ability to be anempathic listener, flexibility, willingness, and adaptability to working withdiverse populations. Must also have the ability to communicateeffectively and possess good time management and organizational skills. CPR and First Aid Certification will be required. Thisposition requires training and certification in and the ability to adequatelyimplement a variety of verbal de-escalation methods and personal/client safetytechniques.Physicalde-escalation techniques will only be implemented at McNabb Center facilities,and not in community settings. F endorsement is required for occasional transportation of clientsas needed. The program will partner with schools to provide trauma-informedconsultation, education, training, crisis support, screening, assessment, and referral/linkage.This position will also support and train staff to offer debriefing to schoolwhen suicides or community violence occurs. Staff provides case managementservices to children ages 4-21 with a mental health diagnosis, as needed. Staffprovides crisis intervention and emergency services, as needed. Interfaces withthe school system and other agencies to increase cooperation and consistencybetween home, school, and community. Utilizes center wide and communityservices as needed for client benefit. Embraces the key values of case management: empowerment, normalization,rehabilitation, and continuity of care.Summary of role ofteam: The school based team serves as a treatment team to plan, implement,and evaluate successful interventions for children and families. Works alongside schools to identify andcollaborate about mental health services to selected schools while providinglinkage and coordination to appropriate services and training, as requested._____________________________________________________________________________________Typical Working Conditions/EnvironmentJob duties are primarily performed in the community setting,but services are also provided in the school setting, homes, and office. Case Manager is present and readyto begin session at the appointment time, every session.b. Case Manager calls to reschedule any sessions that he/shewill be unable to attend or that the consumer missed or canceled 100% of thetime.c. Case Manager communicates accurate and thoroughinformation to outside agencies 100% of the time when clients need different ormore appropriate care that HRMC cannot provide.d. Case Manager contacts are completed each month, accordingto program standards.e. Communicates to Team Leader in weekly supervision ifreferrals become low to ensure adequate and timely caseload assignments ofclients on referral listf. Meet weekly with direct supervisor to discuss clinical andadministrative concerns/changes.* Documentation requirements* Ensures all family/client participation in tx planning as evidence by signatures on tx plans.* Create and implement treatment interventions that are reflected in progress note documentation based on the treatment plan goals* Respond to all flags and e-mails within 2 business days* Respond to all voice mails within 30 minutes* Attend monthly treatment teams and staffing meetings with Director/Program Coordinator/Team Leaders.* Attend and participate in school based meetings with school personnel and community partners involved in the community schools.* Complete all required clinical documentation in accordance with Center Policy and Procedure and funding source guidelines.* Update treatment plans, crisis plans, consent to contact, DLA-20 and updated admissions signature page every 6 months* Complete and submit productivity II summaries to Team Leader by the 3rd business day of the month.* Complete initial treatment plans within 30 days of intake* Create daily progress notes and sign in EMR within one week of service provided* Submit all daily contact logs monthly.* Submit hand generated productivity to Team Leader by the third working business day of the month.* Creates and implements at least 1 measurable treatment intervention per treatment plan goal.* Utilizes family strengths and empowers the family to implement effective interventions as evidenced by the client's progress toward goals.* Makes referral to less intensive services upon anticipation of d/c.* Leadership* Develops and implements training topics/presentations that support enhancing trauma-informed environments in schools.* Facilitate debriefing activities in schools when a crisis team is requested.* Supports the crisis response team by implementing trauma-informed interventions and providing referrals for ongoing care that support student and family needs.PIdba0de246acc-37395-#######0
