Role Overview:
As an Assessment Nurse, you will:
- Perform independent home visits to assess functional, psychosocial, and environmental safety needs.
- Evaluate eligibility for services and develop individualized care plans to help members safely remain at home.
- Communicate effectively with culturally diverse members, caregivers, and providers.
- Leverage time management skills to prioritize and deliver high-quality care.

Qualifications:
- License: Current NYS RN license and registration.
- Education: Associate's degree required; Bachelor of Nursing degree preferred. Bilingual BSN new graduates are encouraged to apply.
- Experience: Minimum one year of experience in home care, acute care, or community health nursing preferred.
- Knowledge: Familiarity with Medicare, Medicaid, and NYS DOH standards and regulations.
- Languages: Bilingual in English and any of the following is highly desirable: Spanish, Mandarin, Cantonese, Korean, Creole, Bengali, Hindi, or Arabic.
