*Job Description:*We are seeking a compassionate and detail-oriented *UAS Assessment Nurse* to join our team in the Bronx. In this role, you will conduct *Uniform Assessment System (UAS)* evaluations to determine eligibility and care needs for individuals requiring home care services. This is a vital position that requires excellent clinical judgment, professionalism, and communication skills to ensure accurate assessments and care recommendations.*Key Responsibilities:** Conduct in-person *UAS assessments* for Medicaid beneficiaries in compliance with state guidelines.* Perform comprehensive evaluations of patients' medical, functional, and social needs.* Utilize the NYS Uniform Assessment System (UAS-NY) software to document findings.* Develop person-centered care plans based on assessment outcomes.* Communicate findings effectively with patients, families, and healthcare providers.* Maintain compliance with all state and federal regulations.* Collaborate with the care management team to ensure timely assessments and continuity of care.* Participate in ongoing training and quality assurance reviews.*Qualifications:** *Registered Nurse (RN)* license in the state of New York (active and in good standing).* Experience conducting *UAS assessments* or similar clinical evaluations preferred.* Strong clinical knowledge of geriatric, chronic illness, and disability care.* Proficiency with the *UAS-NY system* and electronic health record (EHR) systems.* Excellent organizational and time management skills.* Strong interpersonal and communication skills.* Ability to travel within the Bronx as needed.Job Types: Full-time, Travel nursingPay: $85,000.00 - $90,000.00 per yearMedical Specialty:* Home HealthAbility to Relocate:* Bronx, NY 10499: Relocate before starting work (Required)Work Location: In person
