Our employees make all the difference in our success!The Unit Secretary provides administrative and health information management support to the Inpatient Unit (IPU). Responsible for daily reporting of all patient status changes, database/spreadsheet updates, and assembly and analysis of all active and closed medical records.Position Requirements:
- At least 2 years of medical secretary or medical/hospital office expereince required.
- Switchboard experience preferred.
- High school or diploma required.
Essential Duties and Responsibilities:
- Accurately reports and maintains databases and spreadsheets for daily patient status and census.
- Arranges in a timely and effective manner diagnostic tests for patients as requested.
- Maintains a master file and supply of essential forms for the operation of the IPU.
- Maintains and updates a log of potential admissions.
- Conducts daily audit of all active patient charts ensuring that State Regulations and CHAP standards for documentation are met.
- Conducts detailed analysis of closed charts for submission to Central Medical Records ensuring that State Regulations and CHAP standards for documentation are met.
- Maintains par level of all supplies for IPU staff to facilitate quality patient care.
- Assists with the assembling and tracking of admissions paperwork to establish a medical record for each patient.
- Audits admission documents for timely and completeness as follow-up to Executive Director / Office Manager / Clinical Supervisor audits: includes, completion of Consents; Elections of Benefits; Financial Agreements; Certifications of Illness by the Attending physicians and Hospice Medical Directors; Letters of Medical Necessity; noting acceptance or non-acceptance of Standing Orders as properly signed and dated; and other documentation as required.
- Completes the tracking and timely processing of medical records for discharged and deceased patients by concurrently auditing and notifying staff of any deficiencies.
- The medical records are to be audited and sent to the Central Health Information Office within three weeks of discharge or death, as it is a requirement that medical records are complete within 30 days.
- Completes the timely alphabetizing, collating, and filing of all submitted paperwork into the medical records.
- Assists the HMIC Medical Director, Clinical Supervisor, or Office Manager to prepare paperwork for team meetings and for the Medical Director as needed.
- Participates in keeping the medical records in an orderly and secure manner in accordance with accepted standards of (CHAP Security and Privacy Rules) practices.
- Completes all requests for information following policies and procedures, per CHAP regulations.
- Participates in quality improvement, assurance, and compliance activities for audits, surveys, and projects, as needed.
- Assists with medical record audits to assure accuracy and timeliness for Billing Holds and Billing Releases.
- Effectively and courteously handles phone calls to the HMIC.
- Directs, takes, and relays messages to the appropriate person.
- Courteously and effectively interacts with staff, volunteers, patients, families, and visitors to assure hospice mission is achieved.
- Provides information regarding unit operations to visitors; provides direction to different areas of the unit, coordinates interactions between visitors and staff.
- Interfaces with outside medical service providers / vendors, (funeral homes, ambulance, etc.) reference patient status and information.
- Assists Clinical staff with medical record reviews and requests.
- Achieves expected productivity standard as defined by the organization and in alignment with best practices/the Capital Caring Health model. Reviews admission documents to ensure all financial documentation has been accurately obtained.
- Sends copies of all financial paperwork as required by Patient Accounts on a daily basis.
