Position Overview

Provides financial evaluation and case management services to eligible veterans and their families to assist them in achieving economic self-sufficiency. Eligible veterans include veterans seeking financial assistance and veterans receiving VA Pension or other social service payments.

Summary of Job Duties

Employees are expected to perform essential duties to performance standards. As needed, reasonable accommodations will be made for otherwise qualified employees with disabilities that are covered by the Americans with Disabilities Act (ADA), in so far as the accommodation does not create an undue hardship on the Montgomery County Veterans Service Commission.

Duties include but are not limited to the following:

* Leading staff in the creation policies and implementation of best practices to build a program to assist indigent veterans toward self-sufficiency. This includes developing a financial incentives program.

* Reviews clients request for financial assistance and helps to create a tailored care plan to meet client's needs and issues by: o Assesses client's financial, self-sufficiency and support network situation through one on one in-person meetings.

* Assisting clients with identifying and managing barriers to achievement of healthy self-sufficiency.

* Coaching clients to set individualized goals and provides support and feedback as the client works toward achieving their goals.

* Connect clients to relevant community resources, with the goal of enhancing patient health and well-being, increasing client self-sufficiency and reducing health care, housing and utility costs

* Coordinate as needed with client supports, community resources, treatment providers, and VSOs to facilitate client success

* Works as a coach to collaboratively work with the client to set personalized goals, as well as supporting, motivating, and providing feedback to clients as they pursue their goals.

* Seeks and fosters partnerships with community services providers who will assist in developing independence strategies that will move veterans toward self-sufficiency.

* Conducts periodic home visits to assess family resources and needs.

* Maintains confidentiality of confidential and sensitive information.

* Performs job responsibilities in accordance with the Commission's policy, procedures and contracts as applicable, and in compliance with Federal, State and local laws and regulations.

* Keeps abreast of current trends and practices in the field. Attends and participates in required training on topics relevant to the job.

* Works assigned schedule, exhibits regular and punctual attendance and works outside of normal schedule as required.

* Promotes timely access to care and community resources, utilizing resources to remove barriers to care

* Increase utilization of preventive care with motivational interviewing techniques

* Document in developed care plan and communicate with director client progress toward goals

* Increase continuity of care by managing relationships and communication with tertiary care providers and referrals while maintaining a data base of contacts, programs and referral processes of each care provider

* Build rapport, maintain client contact, monitor and track client progress

* Maintain monthly statistics of cases handled and create reports to Director and Commission

* Assist in educating and training staff in areas of social services and resource providers

* May collect financial information and supporting documentation necessary for a financial application and determine need.

* Verifies housing costs and utility costs, reviews income and expense information, bank activity and client file history to make recommendation for appropriate assistance for community and in house programs.

* Maintains the highest level of integrity and professionalism in all aspects of job performance.

OTHER DUTIES AND RESPONSIBILITES

Performs other duties as assigned including community outreach events when appropriate.

SCOPE OF SUPERVISION

None, but will work closely with Veteran Service Officers.

EQUIPMENT OPERATED

Computer, telephone, other general office equipment. Personal vehicle and Commission vehicle.

CONTACTS WITH OTHERS

Clients and family members, Dayton VA Medical Center staff, social service agency representatives, vendors, coworkers, union representatives, commission members, public and other internal and external job contacts.

CONFIDENTIAL INFORMATION AND DATA

Social Security numbers, private addresses and phone numbers, confidential information made available through VA software pertaining to both client and non-client veterans, award letters, income records, and other confidential or sensitive information.

RECORDS

Client Veteran financial and service files.

WORKING CONDITIONS

Good office working conditions when working in the office. When working in the field, the employee is exposed to those field conditions. The employee may have to transport clients to community resources, to include the Dayton VA Medical Center.

USUAL PHYSICAL DEMANDS

The following demands are typically used and exhibited by employees when performing this job's essential duties and responsibilities. Rather, they are illustrated to help the Commission, and an otherwise qualified employee or job applicant with an ADA disability, identify essential job duties that need to be reasonably accommodated, and the type of appropriate reasonable accommodation which may be available which does not create an undue hardship on the Montgomery County Veteran Service Office.

The employee frequently converses verbally with others in person and by telephone. The employee regularly exhibits digital dexterity and eye-hand coordination when performing general typing, word processing, data entry, paperwork and other similar tasks. The employee frequently sits for extended periods of time, and frequently walks. Vison demands include close, relatively detailed vision when using a computer screen, and performing other tasks. The employee occasionally sits in a vehicle for periods of time while driving to and from various locations.

Minimum Qualifications and Requirements

In accordance with Ohio Revised Code 5901.06, employee assigned to this job shall be an honorably discharged Veteran. Must submit DD214 with application.

Applicable laws or regulations require that employees assigned to some jobs attain a specific level of education, experience, or training, and license or certification. For other jobs, a combination of various levels and types of education, experience, training, and license or certification may qualify a person to perform essential duties of this position. Qualifications specified for this position are:

Education, Experience and Training

Bachelor's degree in social sciences and experience working with clients in cycles of poverty, living with housing and food insecurity or other socioeconomic barriers to self-sufficiency. A valid driver's license. Candidates without a bachelor's degree, but with lived experience and/or extensive experience with this population may be considered. Social Work, Counseling, and/or Chemical Dependency licensure is a bonus. 3 to 5 years working experience in community health, social work or case management role preferred.

Each employee assigned to this job shall be an Honorably Discharged Veteran, or if a qualified Honorably Discharged Veteran is not available, the spouse, surviving spouse, child or parent of an Honorably Discharged Veteran may be assigned.

A qualified candidate will have the ability to develop and maintain skills in time management and organization as well as work independently and dependently in a team-driven environment. An ideal candidate will exercise sound judgment and decision-making and can handle sensitive information and inquiries from contacts with officials and the public.

Licenses or Certifications

State Motor Vehicle Operator's License.

Additional Qualifications

Upon conditional hire into this position, and as a condition of continued employment, each employee is required to submit to and pass a drug test, a NAC background check, and be insurable under the Commission's driving insurance provider guidelines.

Ability to document personal identity and employment eligibility within three days of original appointment as a condition of employment in compliance with Immigration Reform and Control Act (I-9) requirements.

Employees are provided the opportunity to participate in training and education programs. Some training programs may be mandatory, and others are optional. These opportunities may include on-the-job training, self-study, seminars, in house programs and in service programs.
