We are seeking a dedicated and experienced Nurse Practitioner to join our dynamic team. This role will be pivotal in assessing patients health status and providing care plans while utilizing cutting-edge technology to monitor and improve the health outcomes of our long-term care patients. As a Virtual Nurse Practitioner with Circadia Health, you will play a crucial role in our mission to enhance patient care through innovative remote monitoring solutions. You will be responsible for performing the initial patient visit, developing individualized care plans and collaborating with our interdisciplinary team to ensure the highest standards of care.Responsibilities:Perform comprehensive assessments of patients using telehealth platforms to gather medical histories and evaluate current health conditions.Create individualized care plans that specify health monitoring parameters and outline achievable health goals for each patient.Maintain accurate and timely documentation of patient interactions, assessments, education, and care plans within the EHR system. Ensure all records are compliant with policies and regulatory standards.Assist in training other providers on remote monitoring workflow and best practices.Collaborate with Skilled Nursing physicians, Circadia physicians, nurses, and other healthcare professionals to provide a comprehensive approach to patient care.Participate in multidisciplinary team meetings to discuss and coordinate patient care strategies.Qualifications:Certified and licensed as a Nurse Practitioner in CaliforniaAdditional state licenses including but not limited to NY, NJ, or MI is a plusMedicare participation and ability to have company bill for services on your behalfCertification from ANCC (or equivalent) as an Adult, Family, Geriatric and/or Acute Nurse Practitioner1+ years of Nurse Practitioner Experience working with complex adult populations1+ years of telehealth experienceAbility to practice independently with little or no supervisionComfort using technology such as Google Suite, multiple EMRs, SlackComfortable in a dynamic and always evolving start up environmentEnglish required, Spanish preferredBenefits:Full healthcare coverage with Anthem BCBS (or similar) with dental and visionWellness programs, such as gym membership reimbursement or access to virtual fitness classesMental health resources, such as access to counseling or meditation appsContribution to 401(k) retirement planPaid Time Off (PTO) accrual policy of 1.7 working days per monthCollaborative environment, working directly with designers, engineers, scientists, and clinicians$120,000 - $140,000 a year
