Regency Home HealthCare has immediate opportunities for compassionate people to join our team! The RN Case Manager provides a combination of direct care by performing hands-on nursing, advocating for, and collaborating with clients & their families, and keeping communication open with the entire healthcare team. Represent the companys values while maintaining a professional working relationship with others. Perform a combination of hands-on care for medically complex clients on skilled nursing visits. Advocate for clients and assist them in achieving their goals and independence. Follow all guidelines and remain in compliance with documentation and policies.
Requirements/Qualifications:
REQUIRED: Hold current RN license in the state of Minnesota and Wisconsin
REQUIRED: Hold current CPR certification
Possess excellent leadership & communication skills as well as attention to detail
Demonstrate consistent integrity, good judgment, and initiative
Must have basic computer skills and the willingness to learn new EMR software
Essential Functions/Areas of Accountability:
Work hands-on with clients
Manage supervise staff
Ensure the day-to-day operation of group settings
Perform visits and Case Management in individuals homes
Advocate for clients physical and mental well-being
Perform assessments and monitoring
Help maintain a positive and professional environment
Be a resource for staff and assist as needed while proactively promoting teamwork
