About the Job:
Provides professional physical therapy services to clients/patients of all ages and families whose needs change from basic to complex and are rapidly changing. Plans and implements age-appropriate specific treatment programs for individual client/patients with a level of skill in accordance with accepted principles and practice.
What You'll Do:
* Plans, develops, and implements physical therapy care plan appropriately.
* Regularly re-evaluates and updates appropriateness of care plan and physicians' orders.
* Communicates appropriate information to patients, physicians, peers, managers, physicians, departments, and other agencies involved.
* Integrates problem solving methodology and quality improvement in resolving problems related to patient care and organizational processes.
* Orients and supervises PTA's according to State of NH guidelines and VNA policy.
Who You Are:
Education: Degree from an American Physical Therapy Association (APTA) accredited program required.
Experience: Minimum one (1) year of experience in health care setting performing similar work activities preferred.
Certification/Licensure: Licensed as a Physical Therapist by the state of New Hampshire Office of Licenses Allied Health Professionals required. BLS, CPR required.
Software/Hardware: Documentation on laptop computer, basic typing skills.
Schedule: Participate in rotating weekend commitment.
Other: Valid Driver's License required to travel throughout southern New Hampshire to provide home health care service and other job-related activities required. Proof of auto mobile insurance required, and minimum coverage met. Motor vehicle background report will be conducted.
