Based on those identified needs, the RC develops a coordinated services plan, connects the individual with appropriate service providers, offers care coordination support/assistance, and monitors for the successful resolution of the identified needs. Additionally, the RC serves as the region team lead and supports team members with care coordination consultation, regional case triage, resource development and both regional training related to resource development and supportive services best practices.Minimum QualificationsDemonstrated knowledge and familiarity with the military services, culture and/or military experience. Demonstrated knowledge of suicide interventions and mandatory reporting requirements. Demonstrated ability to communicate (written and oral) clearly and effectively. Experience in public speaking and delivery of subject matter trainings. Ability to engage SMVF population, develop professional relationships, and advocate for thoseindividuals receiving services.Demonstrated ability to coach, motivate and lead teams while managing a high volume of work.Demonstrated knowledge of human services, behavioral health, rehabilitative, and other support service systems for veterans and their families.Ability to work collaboratively with a wide range of federal, state and local officials including public and private providers in serving the SMVF community.Additional ConsiderationsWork experience in human services and/or behavioral healthcare field required. Related field or combination of education, training, or experience may substitute.Education and or experience (in social work, psychology, human services, education, or an approved field) preferred. Related field or combination of education, training, or experience may substitute.Military/veteran/family related life/work experience preferred.
