About Council on AgingCOA is a nonprofit organization dedicated to enhancing quality of life for older adults, people with disabilities, their families and caregivers. We promote choice, independence, dignity and well-being through a range of services that help people remain in their homes for as long as possible. COA is a state-designated Area Agency on Aging, serving a 5-county region. The
Waiver Support Specialist ensures that members receive scheduled services and their needs are met, while providing necessary resources and education to help them remain in the least restrictive environment.
Key Responsibilities: - Establish and maintain positive relationships with members, encouraging them to utilize services to their optimal benefit.
- Complete home visits to review ongoing services and care plans for identified needs.
- Provide information on community resources and agency services, making referrals as needed.
- Collaborate with the Interdisciplinary Care Team to meet member needs.
- Monitor service delivery for effectiveness and ensure member/caregiver satisfaction.
- Empower and motivate members to access health and community resources through education, outreach, home visits, mentoring, and referrals.
Qualification Requirements: - Bachelor's degree in psychology, social work, gerontology, sociology, or a related field, with at least one year of experience working with persons with disabilities/chronic conditions and Long Term Supports and Services (LTSS).
- Good working knowledge of issues related to the elderly and disabled populations.
- Excellent interpersonal communication skills and the ability to adapt to frequent change.
- Proficient computer skills including MS Word, MS Excel, and data entry.
- Valid driver's license and reliable transportation for travel within Butler, Clermont, Clinton, Hamilton, and Warren Counties.
