General Description:

OneFifteen is a non-profit healthcare ecosystem dedicated to the full and sustained recovery of people suffering from drug addiction. OneFifteen has developed a tech-enabled system of care that offers a treatment center, rehabilitation housing, and wrap-around services, all on one state-of-the-art campus. OneFifteen deploys a learning healthcare system to advance our understanding of addiction medicine and the best strategies to promote sustained recovery. Our ultimate goal is to support individuals to live healthy, fulfilling lives free of addiction. As such, we will focus on a patient-centered experience and ensure a patients whole health needs are fully met, including biopsychosocial needs.

The Chemical Dependency Counselor will collaborate with other team members to provide best-practice care with an innovative spirit and help drive a holistic approach to care and reduce stigma. At the same time, the Chemical Dependency Counselor will be comfortable working in ambiguous situations and blend best-in-class care delivery models with advanced technology solutions to engage care team members, improve outcomes, and reduce costs.

Duties and Responsibilities:
  • Conduct individual and group therapy sessions to address substance abuse issues.
  • Provide case management services, including client assessments and treatment planning.
  • Assist clients in developing, implementing, and evaluating their own treatment goals
  • Monitor and document client progress and maintain accurate and up-to-date treatment records.
  • Collaborate with other healthcare professionals to ensure coordinated care.
  • Participate in interdisciplinary team meetings and communicate effectively with team members.
  • Facilitate peer support groups to promote client accountability and community involvement.
  • Complete all documentation in the required time frames.
  • Assure documentation is completed per OMHAS, regulatory, and agency requirements following best practices.
  • Complete documentation within a specified timeframe using Electronic Medical Records.
  • Attend required meetings, including individual and group supervision, treatment team meetings, training, and community events as appropriate.
  • Other duties as assigned.
Knowledge and Skills:
  • Excellent verbal and written communication skills; strong teamwork and organization/time management skills.
  • Excellent interpersonal skills.
  • Knowledge of community agency and resources.
  • Computer and typing skills. Must be able to use an electronic health record system.
Education and Experience:

Minimum:

  • LCDC II or III required
  • A minimum of 2 years of experience working with chemically dependent populations.
  • Knowledge of substance abuse treatment models.

Preferred:

  • Active affiliation with appropriate professional networks and organizations and ongoing community involvement are preferred.
Work Conditions and Physical Requirements:
  • Work Setting: Indoors, environmentally controlled
  • Body Positioning: Spend time sitting and standing, making repetitive motions, spend time using hands to handle, control or feel objects, tools or controls. Frequently lift or move up to 10 lbs. and occasionally lift or move up to 50 lbs.
  • Communication: Contact with others, electronic mail, in-person discussions, public speaking, telephone, video
