Job Description:
Position is full time in office (40 hours)-4/10 hr. shifts. Schedule is every Saturday and Sunday. The other 2 days are flexible.The Weekend Intake Coordinator is responsible for the processing of all incoming patient referrals. Core responsibilities include, but are not limited to, inputting referrals, verifying payer source and eligibility, gathering medical records, responding to phone calls in a timely manner, preparing admission paperwork, scheduling admission appointments, following up on pending referrals and processing CTIs. Position is full time and weekends are required. EDUCATION AND/OR TRAINING REQUIRED:High School Diploma required; College Degree preferred.EXPERIENCE/SPECIFIC SKILLS AND ABILITIES REQUIRED:Excellent oral and written communication, clinical and administrative skills. Able to work collaboratively in a positive fashion with other health care professionals. Able to prioritize varied projects. Dependable, flexible and tactful.PRIMARY RESPONSIBILITIES:
  • Manages referrals throughout the entire intake process, including retrieving referrals from the fax server or triage calls.
  • Ensures that all initial certification of terminal illness (CTI) forms are sent to the appropriate physicians, tracks the receipt of forms by working collaboratively with internal staff and external customers, and ensures that forms are returned and processed in a manner that meets state and federal regulations.
  • Works cooperatively with the clinical department in providing continuity of information for patients and staff.
  • Uploads admissions documentation into the electronic medical record.
  • Process financial and demographic information through appropriate Medicare and/or Medicaid systems to determine eligibility and the correct hospice benefit period and documents information correctly in the electronic medical record.
  • Creating accurate admission paperwork to send to the admission nurse or the patient or representative.
  • Follow-up on pending referrals, non-admits, and discharges.
SECONDARY RESPONSIBILITIES:
  • Assists with incoming and outbound phone calls handling each call with a high-level of customer service and professionalism.
  • Communicates with patients, families, referral sources and other SFR departments to support patient care needs
