JOB REQUIREMENTS Minimum Education: Graduated from an accredited school of nursing who is licensed in the state in which practicing. - Minimum Work Experience: Minimum 2 years of experience as a registered nurse, preferred. ESSENTIAL FUNCTIONS1.Answers call for Home Health and/or Hospice after hours and/or weekends.Triages patients and provides follow up PRN visits as needed.Provides services in accordance with the patients plan of care. 4.Makes the initial evaluation visit when requested. Initiates the plan of care and necessary revisions. -5.Contacts the patient provider and initiates appropriate nursing procedures as ordered.6.Provides appropriate and timely documentation in the patients EMR.7.Coordinates services with the IDG team. - -8.Informs care team members of changes in the patient's condition and needs of the patient.9.Counsels and educates the patient and family/significant others in meeting patient's needs.10.Completes certification/recertification orders and discharge summaries on a as needed basis.11.Refers patient to other provided services as needed.12.Participates in peer review and Quality Assurance and Performance Improvement as assigned.13.Assist in provided other on-call nights, weekends and holidays, when needed.14.Participates in and presents in-service programs.15.Understands and adheres to established Agency policies and procedures.If providing on call service during the weekend, participates in the Monday AM call report meeting. -NON-ESSENTIAL FUNCTIONSTreats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families. Functions as liaison between administration, patients, physicians and other healthcare providers.Maintains a good working relationship both within the department and with other departments. Works together as a team to provide safe, effective, quality care to all MUSC patients.Manages and operates equipment safely and correctly.
