Wellness Resource Coordinator VISTA
The Center in Hollywood provides a safe space for people experiencing homelessness to connect to community and practical resources such as a mailing address, food, showers, and clean restrooms as well as healthcare and housing. In 2019, in partnership with Saban Community Clinic (partner FQHC), The Center opened the on-site health clinic, called the Community Wellness Clinic, with the capacity for more than 25+ primary care appointments per week. The clinic serves as a referral point for emergency health needs as well as other medical interventions for people experiencing homelessness in Hollywood. 50% of our participants identify as people of color, 14% identify as transgender, and over 65% are Chronically homeless, meaning they have lived on the streets for a year or more (often, for over a decade). The Community Wellness Clinic provides a safe and extremely low-barrier space for those who are among the most vulnerable in Hollywood, Midtown, and the surrounding neighborhoods. Last year, The Center provided more than 12,512 hours of case management and clinical service with more than 100 people moving into interim or permanent housing (housing is health care!) and facilitated nineteen unique support groups offered on-site. Member Duties : The Wellness Resource Coordinator VISTA will serve in the Community Wellness Clinic. The VISTA will focus on leveraging resources surrounding clients that come to The Center for daily group programming through our day center and street outreach, with the goal of increasing connectivity in this population. In particular, the VISTA will develop more efficient ways to link these clients to primary care, behavioral health care, specialist, and dental care. We believe that this role is pivotal to providing care for clinic clients by ensuring healthcare systems and delivery methods remain current, evidence-based, and responsive to the needs of the unhoused community.
