Job Description:
A Brief Overview This role is responsible for delivering Whole Person Health coaching and well-being programming through UH Population Health. This includes providing individual and group coaching sessions for patients and caregivers, leading education and skills-development that aligns with their values and result in meaningful health outcomes. Group well-being programming on a range of health topics will be delivered to patients and caregivers to empower and equip all those who interact with the UH health system. What You Will Do
  • Contact and schedule health coaching sessions with eligible patients and caregivers.
  • Conduct individual coaching sessions following the Whole Person Health coaching model via in-person outpatient, telehealth, and email messaging modalities.
  • Facilitate the exploration of personalized mission reflection, personal health inventory creation, and action planning processes with patients and Whole Health group sessions.
  • Provide individual and group education and evidence-based interventions related to lifestyle behaviors and health management including but not limited to diabetes management, hypertension, nicotine cessation, exercise, nutrition, and mental health.
  • Co-develop plans for health behavioral changes that increase quality of life and reduce risk of chronic disease diagnosis or progression.
  • Perform coaching sessions with compassion, empathy, and cultural competence to meet patients where they are and help them develop relevant health action planning steps.
  • Utilize positive inquiry, motivational interviewing, reflective listening, and other wellness coaching strategies to elicit realistic and applicable lifestyle goals and action steps.
  • Be familiar with UH Connor Whole Health services, UH system wide well-being provisions, and UH Employee ACO benefits in order to suggest well-being programs and healthcare services that meet the patient's interests and needs.
  • Connect patients with integrative and specialty providers as applicable through coordination with care management and patient navigation services in order to maintain appropriate scope of practice, enable best practices for treatment, and connect to community/social services to meet patient needs pertaining to social determinants of health.
  • Develop shared goals and action steps related to skills, habits, and health behaviors of interest. Collaborate in interdisciplinary team setting to optimize patient experience and care outcomes. Maintain follow up contact and coaching to provide ongoing support. Conduct group sessions providing education and interventions related to well-being topics including exercise, mindfulness, stress management, nutrition, resilience, and behavior change. Develop and maintain group presentations for well-being topics to deliver to caregivers, patient group appointments, and community members.
  • Support and work closely with well-being coordinators and managers throughout the UH system to implement various well-being related initiatives.
  • Maintain accurate and detailed documentation records of patient progress and outcomes in health system electronic health records database or other databases as required to maintain continuity between sessions and providers.
  • Maintain detailed documentation of group well-being sessions in internal databases as required.
  • Customers•Cultivate an environment of trust, belonging, and responsibility for personal health decisions.•Provide excellent customer service for patients via email, phone, and virtual meetings.•Effectively communicate with interdisciplinary team members to promote positive patient outcomes.
  • Knowledge•Maintain certification credentials and ongoing education pertaining to chronic disease management and well-being lifestyle interventions.•Understand, or be willing to learn, the Whole Person Health system model and process to implement Whole Person Health care.
  • Performance Management•Peer mentor and motivate other health coaches and instructors. •Provides clear points of contact; creates and maintains environment that fosters open, honest, and transparent dialogue.
  • Process•Assists with the expansion of health coaching services into billable integrative medicine services and ACO care management.•Works collaboratively with Clinical Care team to assure appropriate and successful referrals to integrative medicine services.

  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
Qualifications:Education
  • Bachelor's Degree in health promotion, fitness, nutrition, nursing, counseling, or other related field (Required)
  • Master's Degree (Preferred)
Work Experience
  • 2+ years health coaching experience (Required)
  • 1+ years clinical healthcare experience (Preferred)
Knowledge, Skills, & Abilities
  • High standards of integrity, willingness to learn and apply new concepts, strong personal work ethic, creative problem solving skills, and flexible work approach (Required proficiency)
  • Collaborative leadership skills and team management experience (Required proficiency)
  • Notable client service, communication, and relationship building skills (Required proficiency)
  • Ability to function independently and as an interdisciplinary team player in a fast-paced environment (Required proficiency)
Licenses and Certifications
  • Health and Wellness Coach-Board Certified (HWC-BC) upon hire preferred, required within 90 Days
  • Certification in relevant health domain (personal training, yoga, group fitness, tobacco cessation) (Preferred)
