What you will accomplish in this role:

  • Performing history and physical examinations and pap smears as needed.
  • Documenting visits in electronic medical record in a timely manner
  • Educating patients on their health conditions in a way they can understand.
  • Following up with patients more frequently for effective condition management.
  • Utilizing your support team to complete standing orders
  • Ordering diagnostic tests based on assessments, develop treatment plans, prescribe drugs and other therapies as needed
  • Making referrals as necessary, consulting with supervising medical provider when appropriate.
  • Maintaining patient confidentiality
  • Participating in meetings
  • Working collaboratively with other members of the medical team
  • Collaborating with the management team on quality improvement initiatives
  • Having an open mind to feedback, different perspectives, reflection, and growth.

Who you are and your experience:

  • Must have completed an accredited NP program and be registered by the NYS Department of Education.
  • 2 years or more experience practicing as a Nurse Practitioner
  • Experience working with a diverse and under-served patient population a plus
  • Proven ability to work in a team environment
  • eClinicalWorks experience is a plus

Pay: $140,000.00 - $180,000.00 per year (plus performance based bonus)
