Position Summary:The Wound Care Nurse Assistant is a valuable member of the Long Island State Veterans Home's clinical team who assists the Wound Care Nurse in the implementation of interventions related to maintaining the residents skin integrity. The Wound Care Nurse Assistant will be responsible to provide education to all staff, residents and families as it relates to all skin integrity and wound/ostomy care policies, procedures and protocols. Qualified candidates will demonstrate superior communication and organizational skills while adhering to our high standard of excellence through the LISVH mission, values and code of conduct.Under the direction of the Wound Care Nurse, the duties of the Wound Care Nurse Assistant may include but is not limited to the following:Follows Skin Integrity and Wound/ostomy Care Protocols to be consistent with the Nursing Standards of Practice, Wound, Ostomy and Continence Nurses Society and National Pressure Ulcer Advisory Panel recommendations as well as Federal and State Regulations.Provides education to all employees, residents and families related to skin integrity and wound/ostomy care protocols, policies and procedures.Ensures competency of all employees for skin assessment, skin integrity, wound care and ostomy care.Organizes and conducts wound rounds on each unit.Acts as a consultant/resource to the staff on issues related to skin integrity and wound/ostomy care.Documents assessments and updates in the progress notes using the Electronic Medical System.Assists in the development of the residents Comprehensive Care Plan related to skin integrity, pressure ulcers and wound/ostomy care.Participates in the Skin Integrity Committee and documents meeting minutes; as well as participates and reports at other committees as requested.Maintains current knowledge and competence in skin integrity and wound/ostomy care concepts and management by participating in continuing education programs.Obtain and maintain membership/certification as a wound care nurse from a nationally recognized organization.Collaborates and networks with other Wound care Specialists to remain current in wound care trends and practices.Acts as a role model and a preceptor within his/her role for all staff and students.Communicates with other departments related to Skin Integrity and Wound/Ostomy Care.Participates in annual Department of Health and Veterans Administration Surveys, ensuring the units are in compliance with all Federal and State regulations related to skin integrity and wound/ostomy care practices.Qualifications:Required Qualifications:Registered Nurse License, Associates Degree and 4 years post license experience, Strong organizational and communication skills (both oral and written), Detail-oriented with the ability to prioritize and multitask, Self-motivated and works independently, Proficiency in Microsoft Office ProgramsPreferred Qualifications:Bachelor's Degree in Nursing, wound care nurse certification, five years Long Term Care or related experience, two years' experience as wound care nurse in a healthcare facility.Resume/CV and cover letter should be included with the online application.If within the initial review no candidate was selected to fill the position posted, additional applications will be considered for the posted position; however, the posting will close once a finalist is identified, and at minimal, two weeks after the initial posting date.
