Key Responsibilities:
- Assess, plan, implement, and evaluate individualized wound care treatment plans for residents.
- Perform wound care procedures such as debridement, dressing changes, and wound vac applications.
- Monitor and document patient progress, adjusting care plans as needed.
- Educate staff, patients, and families on wound care techniques and best practices to prevent infection and promote healing.
- Collaborate with physicians, nurses, and other interdisciplinary team members to ensure coordinated care.
- Maintain accurate documentation of wound care assessments, treatments, and outcomes in the patient's medical record.
- Ensure compliance with federal, state, and facility regulations related to wound care.
- Participate in quality assurance and performance improvement (QAPI) programs related to wound care management.
- Provide leadership and guidance to nursing staff on wound care best practices.
Qualifications:
- Valid and active RN or LPN/LVN license in PA
- Wound Care Certification (WCC, CWCN, or equivalent) is preferred.
- 2+ years of experience in wound care, preferably in a long-term care or skilled nursing facility setting preferred
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Detail-oriented with strong organizational skills.
