Key Responsibilities:

  • Assess, plan, implement, and evaluate individualized wound care treatment plans for residents.
  • Perform wound care procedures such as debridement, dressing changes, and wound vac applications.
  • Monitor and document patient progress, adjusting care plans as needed.
  • Educate staff, patients, and families on wound care techniques and best practices to prevent infection and promote healing.
  • Collaborate with physicians, nurses, and other interdisciplinary team members to ensure coordinated care.
  • Maintain accurate documentation of wound care assessments, treatments, and outcomes in the patient's medical record.
  • Ensure compliance with federal, state, and facility regulations related to wound care.
  • Participate in quality assurance and performance improvement (QAPI) programs related to wound care management.
  • Provide leadership and guidance to nursing staff on wound care best practices.

Qualifications:

  • Valid and active RN or LPN/LVN license in PA
  • Wound Care Certification (WCC, CWCN, or equivalent) is preferred.
  • 2+ years of experience in wound care, preferably in a long-term care or skilled nursing facility setting preferred
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Detail-oriented with strong organizational skills.
