General Summary of Position

Position Summary

We are looking for a talented nurse who is passionate about Wound Care and Home Health to join our team! You will act as a resource, consultant, and educator in care coordination/case management for patients requiring clinical expertise related to wound care. This position is Monday-Friday with No Nights, No Holidays, and No weekends! WCC or WTA certification is required within 6 months of hire. Previous Home Health RN experience is required!

Key Responsibilities

Acts as a resource, consultant and educator in care coordination/case management for patients requiring clinical expertiserelated to wound care. Provides clinical care, management and oversight to a population of wound patients to support bestpractice and outcomes. Offers input for policy, procedure, competency evaluation, standardization of care, quality assurance andoutcome evaluations of home care patients with wound needs. Supports staff education related to wound patients. Participates inproduct selection and evaluation for patient specialty care needs including review of wound product formulary.

Qualifications

• Diploma, ADN, BSN, MSN, or CNL from an accredited School of Nursing required.
• Home health experience and/or other nursing experience within the last 2 years required, 2 years acute care experience (ICU, CCU, Med-Surg, Oncology, Gerontology) within the last 5 years and 1-year home health nursing experience preferred.
• Active MD RN License or Active Compact State RN and Basic Life Support for Healthcare providers required.
• WCC or WTA certification required within 6 months of hire.
• DL– Valid State Driver's License in good standing with a dependable vehicle for transportation required.

This position has a hiring range of $87,318.40 - $149,094.40
