Key Responsibilities:
Collaborates with the home health interdisciplinary team to review potential wound care patients for the Home Care Program and assess their eligibility for admission.Conducts comprehensive assessments of deep wounds, skin tears, ostomies, severe burns, pressure injuries, diabetic foot wounds, and more. Determines treatment needs and long-term care plans while coordinating with healthcare providers.Develops wound/ostomy care plans that incorporate input from the patient, caregivers, physicians, and peers. Collaborates with the interdisciplinary healthcare team to create a personalized care plan considering physical, financial, and emotional resources.Participates in the development and revision of wound and ostomy management policies for the agency, ensuring evidence-based practices are followed.Provides education and counseling to patients and families on wound care, pressure injury risks, infection prevention, and treatment measures. Educates and directs personnel in delivering patient care.Maintains a professional attitude, complying with departmental standards while fostering a collaborative working environment with staff, physicians, case managers, external vendors, and community partners.Documents clear and concise progress notes reflecting the implementation and response to the care plan.Provides guidance and validation of wound and ostomy skills to field nurses. Conducts field visits to evaluate nurses' skills.Coordinates communication and documentation between patients, the Home Care Program, medical professionals, and referral sources. Participates in case conferences and interdisciplinary meetings.Assists in establishing protocols for optimal clinical outcomes and serves as a clinical expert to field nurses. Evaluates documentation and suggests revisions where necessary.Participates in departmental conferences, quality meetings, and performance improvement initiatives related to wound, ostomy, and continence issues. Assists in data collection for performance improvement.Ensures compliance with state and federal home health regulations and organizational policies. Participates in surveys conducted by regulatory bodies as needed.Reviews case loads and chart audits, developing corrective interventions where necessary. Ensures compliance with billing guidelines and regulatory standards.Performs other duties as assigned, including home visits during times of increased demand or scheduling challenges.
