Job DescriptionJob Title:WOUND CARE SUPERVISORReports To:DONLocation:CALHOUN NURSING HOMEFLSA Status:NON-EXEMPTDepartment:CNHRevised Date:7/6/2023JOB SUMMARY:Coordinates treatment to provide comprehensive wound assessments and plan of care including goals, implementationof treatment, evaluation and outcomes for patients throughout the organization. Provides resident, family and staffeducation regarding wound management, prevention of pressure ulcers, support surfaces, dressing selections and othertherapies. Is a resource person for physicians and clinical staff. Participates in performance improvement plan of theorganization.STATEMENT OF PURPOSE:The Wound Care Supervisor will assess, plan, implement, and evaluate care for a group of patients, supervise assigned nursing personnel, collaborate with other health care professionals, coordinate ancillary staff, demonstrate professional growth and development in maintaining standards for professional nursing practice at the clinical setting, and provide direct or indirect patient care to all developmental ages.WORKING CONDITIONS:General environment: Works in a well-lighted, air-conditioned area, with moderate noise levels.May be exposed to high noise levels and bright lights.May be exposed to limited hazardous substances or body fluids, or infectious organisms.May be required to change from one task to another or different nature without loss of efficiency or composure.Periods of high stress and fluctuating workloads may occur.May be scheduled as needed including overtime.MISSION STATEMENT:QUALITY HEALTHCARE: In our continuing effort to enhance the quality of life for the communities we serve, the Hospital Authority of Miller County is committed to the delivery of superior, safe, cost-effective healthcare through the provisions of education prevention, diagnosis and treatment.JOB SPECIFIC COMPETENCIES:Ability to perform wound assessment, reassessment and management of all patients and per policy.Knowledgeable of the anatomy and physiology and disorders of the gastrointestinal, genitourinary and dermal systems.Utilizes principles of current wound management theory when providing care to resident with acute and chronic wounds.Demonstrates thorough knowledge of staging system of woundsDevelops skin care programs to prevent skin breakdown of resident.Knowledgeable of surgical procedures and postoperative course of residents undergoing bowel surgery.Demonstrates knowledge and skill to develop and implement policies and procedures to manage residents with draining wounds, fistulae and/or tubes; wound prevention, wound care, ostomy care and incontinence.Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age-appropriate care to the resident population served. Able to interpret data about the residents status in order to identify each residents age specific needs and provide care needed by the resident group.Ability to revise plan of care as indicated by the residents response to treatment and evaluate overall plan for effectiveness.Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the resident regarding pain management.Demonstrates ability to perform treatments and provide services to level licensure.Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning, family is included in teaching as appropriate.Provides education to all residents undergoing surgery that result in an ostomy. Provides pre- and post-op assessments and resident/family education.Treats residents and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families. Functions as liaison between administration, patients, physicians and other healthcare providers.Knowledgeable of medications and their correct administration based on age of the resident and clinical condition.Follows the five (5) medication rights and reduces the potential for medication errors.Identifies and addresses psychosocial needs of residents and families; communicates with social service/discharge planner as appropriate.Performs all aspects of resident care in an environment that optimizes resident safety and reduces the likelihood of medical/health care errors.Is a resource person to physicians, nursing and other hospital staff Provides in-services on wound prevention, wound care, ostomy care and incontinence for the nursing staff.Communicates appropriately and clearly to nurse managers, nursing staff and physicians.Consults other healthcare professional/departments as appropriate to provide interdisciplinary approach to the patient needs.Documentation meets current standards and practices.Compiles data for research studies.Integrates research findings into clinical practice.Attends seminars, conferences, continuing education classes in the areas of wound prevention, wound management and ostomies. Participates in professional societies. Stays current with changes, new advances/treatment in wound care.Serves as a liaison between physicians, residents and manufacturers of clinical products.Knowledgeable of the clinical products currently available on the market. Meets with sales representatives, evaluates products; makes recommendations for products//equipment.Operates all equipment correctly and safely.Coordinates and supervises patient care as necessary.PROFESSIONAL REQUIREMENTS:Follows Code of Conduct policy.Adheres to dress code; appearance is neat and clean.Completes annual educational requirements.Maintains regulatory requirements.Maintains patient confidentiality at all times.Reports to work on time and as scheduled; completes work within designated time.Wears identification when on duty; uses computerized time clock system correctly.Completes in-services and returns in a timely fashion.Attends annual review and/or skills fair and department in-services, as scheduled.Attempts to end conversations and other interactions in a positive manner; leaves others with a good impression of the Hospital Authority of Miller County and its employees.Complies with all organizational policies regarding ethical business practices.Communications the mission, ethics and goals of the facility, as well as the focus statement of the department.GUEST RELATIONS STANDARDS:(All guest relation violations are subject to disciplinary action up to and including termination):Always treat others in a friendly, helpful manner.Refers co-workers to proper sources when unable to provide an answer.Interacts with others in a professional and friendly manner.Takes interest in others and always gives full cooperation to fellow workers.Always maintains an open line of communication with other departments.Thoroughly familiar with the hospital and the services it offers.OTHER:Responsibility to Report: It is the responsibility of every employee of HAMC to comply with federal, state and local laws and regulations, as well as, HAMC Policies and Procedures. Duties, responsibilities and activities may change at any time with or without notice.EDUCATION, CREDENTIALS & EXPERIENCE REQUIREMENTS:Current GA Nursing Licensure.Minimum of two (2) years experience in medical/surgical, rehabilitation nursing.Certified Wound, Ostomy and Continence Nurse (CWOCN) or Certified Wound Care Nurse (CWCN) preferred.Current BCLS CertificationGENERAL REQUIREMENTS:Performs all job responsibilities in alignment with the mission and vision of the organization.Performs other duties as required and completes all job functions as per departmental policies and procedures.Maintains current knowledge in present areas of responsibility (i.e., self-education, attends ongoing educational programs).Attends staff meetings and completes mandatory in-services and requirements and competency evaluations on time.Wears protective clothing and equipment as appropriate.GENERAL SKILLS:Ability to communicate in English, both verbally and in writing.Additional languages preferred.Strong written and verbal skills.Basic Computer SkillsPHYSICAL REQUIRMENTS & DEMANDS:Have near normal hearing: Hear alarms/telephone/normal speaking voice.Have near normal vision: Clarity of vision (both near and far), ability to distinguish colors.Have good manual dexterity.Have good eye-hand foot coordination.Ability to perform repetitive tasks/motion.Continuously within shift (67-100%): Standing, Walking.Frequently within shift (34-66%): Bending/Stooping, Pushing/Pulling, Lift/carry up to 20lbs, Lift/carry > 20 lbs. with assistance.Occasionally within shift (1-33%): Sitting, Climbing, Twist at waist, Lift/Carry > 50 lbs. with assistance, Reaching above shoulder.
