Essential Duties & Responsibilities:
- Meet physical and sensory requirements stated below, and be able to work in the described environment.
- Identify and participate in process improvement initiatives that improve the customer experience, enhance work flow, and/or improve the work environment.
- Assess and document resident progress in the medical record.
- Perform all preventive skin care, dressing changes and wound care.
- Monitor turning and positioning of residents.
- Monitor the treatment of decubitus ulcers with appropriate documentation and communication to physician and family.
- Make rounds with physician, as needed, to discuss the care of residents who are currently receiving decubitus or catheter care.
- Assess weekly nutrition/hydration of residents with State I to Stage IV decubitus ulcers and evaluate and initiate additional nutritional intake, as required.
- Assist in resident care planning when resident is currently being monitored by Treatment Nurse.
- Notify physician of new skin problems, changes of condition and when skin conditions heal.
- Obtain changes in physicians orders as indicated by residents progress or lack of progress
- Communicate pertinent information about resident condition and progress to charge nurses, supervisors and Director of Nursing, as indicated.
- Maintain clean, orderly treatment cart and restock treatment supplies.
- Follow Infection Control policies and procedures.
- Administer medications and treatments accurately as ordered by the physician and according to all facility policies and procedures
- Follow facility policies and procedures for assessment, treatment and documentation of skin problems.
- Document pressure sore status weekly or more often as condition changes.
- Assist in monitoring the inventory of medications, medical supplies and equipment to be sure adequate supplies of skin care products are on hand to meet the needs of residents.
- Participate in educational programs for residents and their family members relative to skin care when a resident is being discharged to home.
- Participate in Quality Improvement activities, as assigned

Job Requirements:
- Registered Nurse or Licensed Practical/Vocational Nurse with current state license.
- Minimum of one (1) to three (3) years related nursing experience preferred.
- Must have a current/active CPR certification.
- Excellent technical, assessment, and documentation skills.
- Effective verbal and written English communication skills.
- Demonstrated basic to intermediate skills in Microsoft Word, Excel, Power Point and Outlook, Internet and Intranet navigation.
- Highest level of professionalism with the ability to maintain confidentiality.
- Ability to communicate at all levels of organization and work well within a team environment in support of company objectives.
- Customer service oriented with the ability to work well under pressure.
- Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level of quality and productivity.
- Strong analytical and problem solving skills.
- Ability to work with minimal supervision, take initiative and make independent decisions.
- Ability to deal with new tasks without the benefit of written procedures.

Physical and Sensory Requirements:
- Moderate physical activity:
- Push, pull, move, and/or lift a minimum of fifty (50) pounds to a minimum height of three (3) feet and be able to push, pull, move, and/or carry such weight a minimum distance of fifty (50) feet.
- Standing and/or walking for more than four (4) hours per day.
- Bending and/or stooping for more than one (1) hour at a time.
- Sitting for more than one (1) hour at a time.
- Requires writing and/or computer work with repetitive typing and concentrating on computer screen and/or documents for more than one (1) hour at a time.
