Position Status: per diem. Performs per diem consult visits upon request.

A Wound, Ostomy, Continence Nurse provides nursing visits to hospice patients with wound, ostomy, and continence issues to collaborate with the attending provider and interdisciplinary team on the plan of care as it relates to the scope above.

Essential Duties and Responsibilities for Wound, Ostomy, Continence Nurse:

  • A Wound, Ostomy, Continence Nurse assists in maintaining current knowledge and competence in the areas of wound, ostomy and continence care
  • Keeps abreast of changes and trends affecting nursing care and WOCN Therapy
  • Periodically assists with review of the agency's standing orders as it relates to WOCN therapy, making recommendations in line with up-to-date accepted standards of practice
  • Makes patient visits in accordance with all agency policies and procedures
  • Documents findings in the appropriate EMR forms and clinical notes within the expected timeframe per policy
  • Participates in staff education in collaboration with the clinical education team in the area of WOCN therapy and interventions as requested.

Qualifications for Wound, Ostomy, Continence Nurse:

  • Education / Certification
  • Graduate of an accredited school of nursing with current RN licensure in the State of New York.
  • Graduate of an accredited program for Wound, Ostomy, Continence Nursing with certification.
  • Valid Driver's License, free of serious violations, and vehicle required for use when on duty.
  • Experience
  • One to two years recent nursing experience with WOCN experience preferred.
