The Wound Ostomy Continence (WOC) Nurse is a crucial player in providing expert care to patients with wound, ostomy, and continence needs. This role involves supporting patient care while facilitating progress and transitions throughout their stay in acute care. The WOC Nurse not only provides hands-on clinical support but also serves as an educator, consultant, and clinical expert.

Key Responsibilities:

  • Conduct comprehensive assessments to collect relevant data pertaining to the healthcare consumer's wound, ostomy, and/or continence care needs.
  • Analyze assessment data to identify actual or potential issues related to wound and continence care.
  • Establish expected outcomes tailored to each individual for care plans involving wound, ostomy, and continence management.
  • Contribute to initiatives aimed at enhancing healthcare efficiency while achieving positive patient outcomes.
  • Gather and analyze data, including cultural factors, to monitor and improve the quality of WOC nursing practice.
  • Work collaboratively with the healthcare team to implement quality improvement strategies that enhance patient care.
  • Review and evaluate policies, procedures, and guidelines to elevate the standard of wound, ostomy, and continence care.
  • Assess patient needs and available resources to ensure optimal outcomes in care.
  • Participate in cost-benefit analysis to inform decisions around care, including product selection and resource allocation.
  • Assist patients in identifying and accessing services for wound, ostomy, and continence care needs.

Qualifications:

  • Graduate of an accredited WOC Nursing Education Program with a commitment to achieve tri-certification within one year of graduation.
  • Bachelor's Degree in Nursing is required; Master's Degree in Nursing is preferred or currently pursued with completion anticipated within five years.
  • Current NYS Registered Nurse license is mandatory.
  • 3-5 years of nursing experience is required.
  • Effective communication skills across all organizational levels and with external stakeholders.
  • Ability to foster a collaborative and supportive environment among healthcare team members.
  • Clear articulation of expectations to staff and care team members.
  • Demonstrates accountability and sound decision-making.
  • Strong analytical skills to identify issues, gather necessary data, and draw valid conclusions.
  • Commitment to professional growth through ongoing education.
