Job Description:
Responsibilities of this position include:
  • Facilitation of team planning to include brainstorming of sustainable informal and formal strategies through-out the Wraparound Process.
  • Responsible for data collection, organization and presentation to the team through-out the wraparound process to drive and encourage successful transition planning
  • Ability to assess the functional strengths of the youth, family and all team members while gathering information around the family story and initial conditions
  • lead the team in the development of a plan of care that defines the unmet needs of the youth and family and assists the team in the brainstorming of strategies to help the youth and family in meeting those needs
  • Assist family in identifying and selecting natural supports to build family team
  • Completes 180-day CANS/ANSA re-assessments and judges, based on these data, whether clients remain in need of the current level of care or need to move to a different level of service intensity.
  • Carries a caseload of 10-12 Wraparound Clients
