Job DetailsJob LocationHazel Atlas - Wheeling, WVPosition TypeFull TimeEducation Level4 Year DegreeTravel PercentageUp to 50% Job ShiftDayJob CategoryNonprofit - Social ServicesDescriptionJob Summary: Responsible for the implementation of Wraparound Services utilizing the philosophies of the Wraparound Model that supports children and families in West Virginia communities and collaborate with varying entities to meet the needs of children and families and demonstrate a commitment to the vision of permanency for every child and advocate for youth. This employee will ensure compliance with safe work practices and maintain safe environments in compliance with established agency standards, licensing regulations and generally accepted practices. This employee will work with youth aged 12 to 17 enrolled in the Safe At Home Program and youth ages 3-21 with a diagnosis of a severe emotional or behavioral disturbance that impedes his or her daily functioning (ICD-10). Duties and Responsibilities:
- Accept up to 15 cases for Youth / Family Wraparound Services
- Intensively engage and meet with youth, their families, and the wraparound team.
- Facilitate family team meetings (including meeting arrangements and documentation)
- Engage and welcome family team members. Provide training on wraparound process to family team members
- Arrange for and, as necessary, advocate for services and supports as outlined in the wraparound plan. Work with treatment, support providers and natural supports to customize services to meet the specific needs of the youth and family.
- Track and assess family progress towards team vision, performance measures and system goals. Track client/family progress.
- Work cooperatively with others to meet agency and program objectives.
- Attain knowledge of and adherence to all agency policies and procedures and applicable laws.
- Perform other duties as assigned by your supervisor and assist with short term assignments to ensure the smooth running of the agency and meeting agency needs.

QualificationsRequired Skills/Abilities:
- Willingness ability to travel throughout the State
- Basic knowledge of and commitment to children who have experienced trauma and/or display serious emotional/behavioral health issues.
- Ability to complete Wraparound and WV Safe at Home training
- 24 hours availability to work with irregular hours
- Ability to work independently while adhering to a team environment.
- Ability to work under pressure and multitask
- Strong interpersonal skills. Good written and oral communication skills
- Ability to use Adobe and Microsoft software
Education and Experience: Requires Bachelor Degree in Social Work or human services related field and a minimum of 2 years' experience serving population of focus. Must be Social Work license eligible with demonstrated knowledge of child and family services. Must have valid driver's license and proof of automobile insurance as required by law
