Job Summary

The Youth/Grief Support Program Associate (YGSPA) is responsible for the coordination and administration of aspects of the Grief Support Program (GSP) that pertain to youth. The YGSPA will schedule, plan, and coordinate Camp Erin Santa Cruz; an annual youth grief camp. Additionally, the YGSPA will develop and implement the school-based Grief Support Program. This position also supports the Grief Support department in conducting intake calls, check-in calls and is back up to the Grief Support team as needed and attends weekly GSP staff meetings. The YGSPA demonstrates: sensitivity and compassion for the issues involved with serving patients and families facing a terminal illness, death and bereavement. Occasional weekend and holiday availability required. This position is approximately 20 hours/week.

Camp Erin Coordination:

* Schedule, plan, and coordinate Camp Erin Santa Cruz, with oversight from the Director of Community Programs

* Oversee the recruitment and selection of volunteers and campers.

* Design, oversee, and lead the volunteer training

* Provide oversight to Camp Erin, Santa Cruz through involvement in selection of campers, developing camp curriculum, training and supervising volunteers, and other tasks as needed.

* Oversee and participate in the interviews of potential campers and their parents/guardians to determine a youth's readiness for camp.

* Coordinate all communication with Camp Erin families, donors and volunteers.

* Camp Weekend - oversee all activities and a smooth flow of communication.

* Coordinate volunteer post camp surveys/debrief.

* Plan post-camp reunion party and any other post-camp grief support activities/events.

* Prepare all documentation for camp.

* Work with marketing and other relevant staff on marketing plan.

* Prepare written reports/materials to Moyer Foundation and others funders.

School-Based Youth Grief Support Program

* Responsible for collaborating with school staff and counselors to assess and onboard students seeking grief support.

* Interviewing and conducting intake assessment of youth.

* Provide trainings to HSCC volunteers and school staff related to youth grief support

* Co-facilitation of youth grief support group along with trained volunteers

GSP Responsibilities

* Prepare for and attend IDT for bereavement collaboration as needed

* Available to help with intakes for adult bereaved clients as needed

* Available to help with check-in calls to adult bereaved clients weekly

* Ensure all documentation related to services is completed in accordance with program and regulatory requirements.

* Utilize community referral sources in the event of a community crisis, individual high-risk patient, or community caller that seems to be high risk, including 911 referral if appropriate.

* Maintain appropriate confidentiality.

GSP Program Coordination and Development

* Develop and maintain good working relationships with referral sources, such as schools.

* Maintain and provide oversight for program educational material.

* Ensure client and family satisfaction through the development of evaluations for individual and group clients.

* Responsible for collaborating with other agencies to promote youth program.

* Communicate with Director of Community Programs any unusual occurrences other than routine issues.

* Demonstrate appropriate boundaries with staff, interns, volunteers and clients.

* Responsible for record keeping which includes client files and program statistics.

* Actively participate in GSP team meetings.

* Participate in volunteer orientations and trainings as requested.

* Participate in agency events as requested.

Compliance

* Ensures all HIPAA Privacy and Security Regulations, Medicare Conditions of Participation, State and Federal Laws and all other regulatory/compliance requirements are understood and followed

* Submits verification of MSW degree from accredited school, passes OIG/GSA and background checks

* Submits acknowledgement of auto insurance annually; keeps drivers license current and maintains a good driving record

* Insures TB checks are completed in a timely manner; at least annually

Productivity

* Follows all requirements for obtaining approval for overtime

* Provides concise and timely reports to appropriate team members as required by standards

* Documents in a timely manner, all activities and services provided using electronic medical records systems

* Submits completed time logs within the required time frames

IDT Participation

* Functions as a member of the IDT maintaining open & organized communication with hospice staff, volunteers and supervisor.

* Participates in IDT meetings in a manner that promotes problem solving, care planning and learning.

Quality Improvement

* Participates in continuing education opportunities both within and outside the agency

* Participates in activities that affirm, enhance and develop the highest quality standards for HSCC

* Participates in Quality Assessment & Improvement activities as requested

Special Projects

* Participates in special projects as requested.

Requirements

Education

* Bachelor's degree in health sciences (Social Work, Psychology, Community Health, Recreational Therapy)

Experience

* Minimum of 1 year of experience in a behavioral health setting required

* Prefer previous experience in hospice or other end-of-life care and services

* Prefer previous experience in working with youth in a variety of settings.

* Prefer event planning experience

* Solid experience and/or aptitude with computer systems and electronic medical record systems and charting

Skills Desired

* Strong verbal and written communication skills

* Skilled in tact and diplomacy

* Capable of bringing appropriate level of sensitivity and compassion to patients/families facing terminal illness, death and bereavement

* Ability to complete psychosocial assessments of youth

* Knowledge of youth and developmental considerations related to grief

* Understands how to hold and apply appropriate boundaries for care

* Knowledge of community social work services, resources and operational standards

* Understanding of all applicable laws and regulations related to social work licensing

* Understands and utilizes solid ethics in decision making and activities

* Bilingual in English/Spanish highly desirable

Physical Demands

* Strength to lift and carry materials weighing up to 10 pounds.

* Vision to read printed materials and a computer screen.

* Hearing and speech to communicate in person, over the telephone and in a group or classroom setting.

Work Environment/Environmental Condition

* Must possess mobility to work in a standard office setting and to use standard office equipment, including a computer.
