Coordinated Care Services Inc (CCSI) seeks a talented Youth/Young Adult Peer Coordinator. This position provides direct 1:1 peer support to youth of transition age with, or at risk of developing, behavioral health challenges. This Credible Messenger proscriptively uses lived experience navigating the mental health system to engage transition age youth in formal behavioral health services and other formal and informal recovery support services. This position also works to build the network of credentialed youth peer support providers in Onondaga County.

CCSI is partnering with the Onondaga County Department of Children and Family Services to hire a Youth/Young Adult Coordinator. This position is employed by CCSI and working on behalf of the Onondaga County Department of Children and Family Services.

In this role, you will:

  • Supports youth/young adults with experience navigating behavioral health system in becoming a credentialed peer support provider.
  • Prescriptively use personal experience as an engagement tool
  • Help transition age youth establish creative community connections and assist them in identifying and developing natural supports.
  • Serves as a credible messenger and partners with youth, young adults, and families where appropriate to complete assessment process, explore natural supports, support registration into an educational program, assist with problem solving, and identify solutions.
  • Educate youth, young adults, and families where appropriate on mental health resources and other supportive services and community resources available and complete associated referrals to services.
  • Takes the lead in and or provides team support for navigating complex referral response needs.
  • Provides mentoring support to enhance team orientation, training practice development.
  • Takes presentation lead for community presentations when needed.
  • Designs and runs youth/ young adult support groups.
  • Conducts community-wide training in youth-guided and youth-driven care.
  • Model System of Care (SOC) principles and practices in interactions with families, youth, and other stakeholders
  • Other duties as assigned.
Requirements:

EDUCATION AND EXPERIENCE:

  • Associate degree in human services or related field plus 2 years providing services to youth of transition age, or equivalent education and work experience. (see CCSI's equivalency chart)
  • NYS Credentialed Peer Support Provider or meet current credentialing requirements (18-29 years old having received MH services)
  • 1 year of experience designing and running peer support groups

Additional Requirements:

  • Satisfactory completion of all required background screenings: criminal, Statewide Central Register and driving record.
  • Valid NYS Class D Driver's License
  • Onondaga County residency

Employees may be directly exposed to protected health information /electronically stored protected health information, personally identifiable information, and other patient/consumer/customer data. Employees must abide by the specific procedures and policies outlined by CCSI's Code of Conduct and CCSI's Privacy and Security Policies, New York State regulations, and Federal regulations.
